The loose ends: what did the abandoned hospital have to do with everything? Why is the mountain a spiritual hot-spot? Why is the dark giant continually acting out his death? Who was the woman that Taro saw in the hospital at the beginning of the show in the hallway? If Miyako isn't in the hidden realm, then why do Taro and Hiruta see her in the forest and school hallway? Why did Makoto's mother leave him with his psycho grandmother? What happened to the missing scenes at the intro (kids running from a giant worm spirit in the abandoned town, Miyako or Mizuki wading through water in a storm, Taro outside the Sake brewery in the snow)? The only contradiction to this I can think of is that the old man had never projected prior to that, and I'm not sure what need tengu or gods would have to project to begin with. You would assume they could transcend any plain of existence simply due to their divine nature etc. What do you guys think?
